At the moment, every health and social care organisation that you use has a different set of patient or service user records for you. These records may duplicate information or one record might hold information about your treatment, care and support that another one doesn’t. To provide the best care to you as a patient or service user it is essential that health and social care professionals have access to the most up-to-date information.  The NEW Yorkshire & Humber Care Record is a secure virtual health and social care record. It pulls key information from different health and social care records and stores it in one combined record. This enables health and social care professionals to find all the key information for your care in one place.

Extended Access Service

Ampleforth & Hovingham Surgeries work alongside other GP surgeries in Ryedale to provide services 7 days per week. 

 Ampleforth Surgery regularly provides routine patient appointments outside of core hours: 

  •       Tuesday up to 8pm
  •       Selected Sunday mornings from 8-10:30am 

Patients from Ampleforth & Hovingham Surgeries can also access appointments at: 

Derwent Surgery:

  • Monday, Wednesday and Friday up to 8pm
  • Saturday mornings from 8-10:30am
  • Selected Sunday mornings from 8-10:30am 

Sherburn & Rillington Surgery:

  • Thursday up to 8pm
  • Selected Sunday mornings from 8-10:30am

In order for you to be seen at one of these other practices within the Extended Access Service, you will need to consent to share your electronic medical record so that the team at Derwent or Sherburn will have access to your full medical record.  You will be asked for your consent when you book an appointment and only with your consent, we will set up this share.  Unfortunately, without this consent, you cannot be seen anywhere other than your usual registered practice.

This practice is one of over 260 practices in England contributing pseudonymised data for national research and surveillance.

These data enable continuous monitoring of infections and diseases in the community and is used in ethically approved research.  The RCGP RSC is the main source of information for Public Health England (PHE) and helps with prediction and management of flu outbreaks and pandemics.

Medication Ordering and Collection

Dispensary telephone lines will be answered between 10 am and 5 pm.  An answering machine will be available to record your details and prescription request if you ring between 9 - 10 am and 5 - 6 pm.  We ask patients to give the surgery 3 working days’ notice when ordering their repeat medications.

Fundraising and Donations

Books and DVDs are on sale at both Ampleforth Surgery and Hovingham Surgery. Books and DVDs cost £1 each and all proceeds will go towards new medical equipment for the surgery.  If you wish to donate any books or DVDs please drop them it at reception.

Access

Ampleforth Surgery has ramp access from the car park at the front of the surgery. A handrail is also being installed to make it easier to access the surgery. Hovingham Surgery offers parking right outside the door on a flat surface which you may find easier.
