If you live in our practice area and would like to register with us, please complete a registration form that is available from our reception desks at both Ampleforth and Hovingham.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
You will be registered with the practice rather than an individual GP and will be able to choose which GP you wish to see. When you book your appointment you will be offered a choice of pre-bookable appointments as this allows you to plan your visit. Alternatively, if you wish to see a doctor the same day or within 24 hours for a medically urgent issue there will be appointments available each morning.
All new patients are offered a health check with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further information.
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
